If you have forgotten the password you registered with, please use the FORGOT PASSWORD link from the Login Box located at the top of the page next to CONTACT or on the Login Form located on the 2022 Survey page. A new randomly generated password will be sent to the email address you provided during registration. Once you have successfully logged in with this new password, you can set it to a more friendly easy to remember password in your PROFILE by clicking on your name at the top of the page.

After LOGIN, click on your NAME in the TOP MENU above the Page Links. This link opens the MY PROFILE page, on the left menu choose EDIT LOCATIONS. On the right side of the page, your defined locations will be listed, typically the location(s) you entered when you registered. To add another location, click the ADD LOCATION button to display the ADD LOCATION FORM. Enter a Location Name and the corresponding Affiliate and City, State and Zip Code for that location. Click the UPDATE LOCATIONS button to save the new or modified location information.
If you have already created multiple locations for your company, a dropdown selection box will appear on the 2022 Survey page listing your locations. Choose the location you want to complete a survey for each time you visit the 2022 Survey page.
